Expand HTTP header settings #4
- add doc/Usage/Filter.asciidoc to explain the usage of the filter headers
- add HTTP headers to Profile (section "headers")
- Use request interceptor to apply filter headers, then profile headers
- add insert/delete actions to ProfileEditor
